Brazil woman dies after rope-jumping instructors fail to attach the cord
According to the BBC, a woman in southern Brazil died after performing a bridge rope-jump in which instructors failed to attach the cord to her harness. An investigation has been opened, and local authorities have ordered a fresh review of adventure-tourism safety standards.

According to BBC Latin America, the incident occurred during an adventure-tourism activity in southern Brazil. CCTV reviewed by investigators showed that the instructor team accompanying the woman from the bridge failed to verify that the rope was connected to her harness.
Local authorities have suspended the operating licence of the company running the activity and opened a negligence investigation into the instructors. Police said evidence had been gathered at the scene and witness statements were being taken.
Brazil's tourism ministry said additional inspections of adventure-tourism operators were on the table. The family said in a statement on social media that funeral arrangements were being made and that they wanted accountability.
