3 midlife health factors that could delay dementia by 13 years

A decades-long cohort study has found that people who manage certain health markers well in midlife see dementia symptoms emerge, on average, 13 years later than those who do not. The research tracked thousands of participants from their 40s onward, measuring the long-term impact of factors including blood pressure, blood sugar and hearing health on brain function.
The first factor researchers identified is blood pressure control. In people with untreated high blood pressure during midlife, damage accumulates over time in the small blood vessels supplying the brain, and that damage accelerates cognitive decline. The study found dementia risk dropped markedly among participants who kept blood pressure consistently within target range.
The second factor is early treatment of hearing loss. Researchers found that untreated moderate hearing loss isolates the brain from social interaction and cognitive stimulation, and that isolation accelerates cognitive decline over time. The effect was substantially smaller among participants who used hearing aids.
The third factor is regular physical activity, particularly moderate-intensity cardiovascular exercise. Researchers recorded better-preserved brain volume and stronger performance on cognitive tests among participants who logged at least 150 minutes of moderate activity weekly.
The study's authors emphasize that these three factors show their strongest effect when addressed together rather than individually. Participants who improved only one factor saw an average delay of roughly four to five years; those who managed all three saw delays of up to 13 years.
Researchers particularly stress that timing is critical. The same interventions starting in a person's 70s showed a substantially smaller effect, aligning with prior research showing that most brain changes underlying dementia begin decades before diagnosis.
Experts say the findings carry particular significance for people carrying genetic risk. The study found the protective effect of managing these three factors held even among participants with elevated genetic risk, suggesting dementia is not determined by genetics alone.
Primary care specialists argue the findings should be incorporated into routine midlife health checks. Some doctors suggest blood pressure and hearing tests should be added more systematically to standard checkups for people over 40.
The study's authors acknowledge the findings represent strong associational evidence rather than definitive causation, but say the size and consistency of the effect support calls for midlife interventions to become a public health priority.
As a next step, researchers plan to test a structured intervention program targeting these three factors in a randomized controlled trial, a step that could turn the observational findings into direct evidence of causation.
